Here you can print out some of the Library's Forms. This can be helpful if you need to obtain a form when the Library is closed or when you are unable to reach the school. You can of course also pick these forms up personally at the Library's Main desk. All Forms are in PDF format. To print them out please click a printer icon on your Acrobat Reader toolbar. Textbook order form OPEN Description: This form is for use by the ECP Academic staff who order books and textbooks to support their subjects. Please read the form's instructions carefully. Leaver's statement form OPEN Description: This form is for use by ECP students who are leaving the school and wish for their deposit to be returned. (Each student has been charged a deposit at the beginning of his/her studies.) Please read the form's instructions carefully. Textbook request form OPEN Description: This form is for use by any ECP teacher or student. A teacher uses this form for scheduling the borrowing of class sets of textbooks from the Library (prior to picking-up the class set). This form is also used for teacher approval of issuing a second copy of textbook. A student can fill out this form for the teacher in case the student needs a second copy of a textbook (or simply an extra textbook that is normally not issued to his/her class). The form must be signed only by this subject's teacher. Please read the form's instructions carefully.
